How do I apply for a marriage license?
Application is done in person. Requirements are: *** Please Note: Premarital blood tests are no longer required by the State of Connecticut as of October 2003 *** 1. Date, location of wedding, who will be performing the ceremony, and telephone number of the person officiating at the wedding.
2. The application is completed and both parties take an oath.
3. The wedding ceremony must take place within 65 days of the start of the application process.
4. If under 18, parent or guardians consent is needed.
5. If under 16, consent is needed from the Judge of Probate.
6. Fee: $40.00 (this includes a certified copy sent to the couple after the ceremony if the marriage takes place in Stratford) If the marriage is occurring somewhere other than Stratford, the cost is $30.00. 2. Proof of spaying or neutering must be presented.
3. Pet information is needed - name, address, phone, owners name, type of dog, etc.
4. Fee: $8.00 for altered dog, $19.00 for unaltered dog.

Per State Statute copies of birth certificates are restricted to certain individuals: Person who is subject of birth certificate - parent or guardian, grandparent, if minor - spouse or children, if over 18. For complete list, see Public Act 96-258.
2. Proper identification is required - Driver's license or other photo identification. If photo identification is unavailable originals or photocopies of 2 of the following documents shall be substituted for it:
a. Social security card
b. Written verification of identity from employer
c. Automobile registration
d. Copy of utility bill showing name and address
e. Checking account deposit slip stating name and address
f. Voter registration card
3. Request for copy of Birth Certificate (see below) needs to be completed.
4. "Full" or "wallet" sizes are available.
